Service Description: Health centre providing family medical care and community-based health programs * team of nurses and other allied health care providers

Culturally appropriate programs and services for Aboriginals, combining traditional health and Western medical practices * aims to improve the social supports and conditions that affect long-term health

Community Health Nurse - Provides public health programs, including health education, vaccination and disease prevention services

Community Health Representative - Coordinates health services including blood pressure and blood sugar screening, foot care, workshops on healthy lifestyles, diabetes, smoking cessation and healthy choices for youth

Visiting Health Professionals - Visits made by dentist, doctor, psychologist, elders, healers and foot care specialist on a regular basis
 
Community Support Services Meal services and activities for seniors
 
Environmental Program Monitors and coordinates the West Nile Virus program and water testing * manages other environmental health related issues on reserve only
 
Falls Prevention and Group Exercise Classes for Seniors In-home exercise and falls prevention instruction for older adults and people with mobility issues * aims to improve strength and balance to prevent injury and falls
 
Home and Community Care Holistic services for Chippewas of Nawash Band Members living in the community who have a physical, emotional, mental or social need for assistance * helps people remain independent in their own homes for as long as possible * needs assessments and case management conducted by community nurse
 
Medical Transportation Transportation to medical appointments for on-reserve band members only * book at least 48 hours in advance for local trips * book at least 20 business days in advance with referral documents and confirmation letter or card from specialist for long-distance trips
 
National Native Alcohol and Drug Abuse Program Addictions prevention, intervention and aftercare services and referral to residential treatment programs
 
Youth Mental Health Prevention Program Provides mental health prevention activities in community and school for children and youth living on reserve
