Service Description: |
Offers education, training, and consultation services to organizations and healthcare professionals providing end-of-life care * services can include: Case consult - Perform full case reviews or answer case-specific questions to assist service providers in managing pain and other end-of-life symptoms for their clients * may be done through video-conference or telephone if available and appropriate for the situation * works with the organization and professionals involved to use the consult as a means for building and information exchange or on-going mentorship Topic-specific education - Provide in-services on topics relevant to the organization or professional, content ranging from psychosocial to medical perspectives on end-of-life care Resource sharing - Connect organizations and professionals with existing evidence-based resources for information on end-of-life care * this includes material that can be passed along to clients and caregivers as part of regular clinical practice or used by professionals for self-study System development - Consultants regularly engage with various hospice palliative care committees that focus on local and regional planning, clinical issues, advocacy, and integration with other sectors of the healthcare system Interdisciplinary Education - Consultants support standardized palliative care education with the goal of increasing the number of professionals and volunteers that receive advanced training and certifications in end-of-life care * courses include:
